摘 要：
随着我国健康医疗模式的转变，越来越多地走向精准医疗，护士也由过去的疾病为中心，变为以患者为中心的精准护理模式。本研究秉持患者导向原则，探讨精准护理护士在此模式下的角色定位与跨学科协作模式。我们通过对多病例分析和实地护理经验交流，挖掘出精准护理护士的角色定位多元化，包括患者关系管理者、跨学科协同者、健康咨询师等。同时，我们提出了跨学科协作模式，其中通信、协商、共享和学习是提升护理服务效率的四大元素。研究结果显示，精准护理护士在跨学科协作模式下，能整合更多资源，提供从预防、干预到复原的全过程精准护理，从而提高患者满意度和降低医疗成本。同时，这种模式也将促使护理专业的进一步发展和优化，为患者提供全方位、全过程和精准的护理。

Abstract:
As China's health care model transitions towards precision medicine, nurses are shifting from a disease-centered approach to a patient-centered precision care model. This study adheres to patient-oriented principles and explores the role positioning of precision care nurses and the interdisciplinary collaboration model under this framework. Through multiple case analyses and field nursing experience exchanges, we uncover the diversified role positioning of precision care nurses, which include patient relationship managers, interdisciplinary collaborators, and health consultants. At the same time, we propose an interdisciplinary collaboration model wherein communication, negotiation, sharing, and learning are the four key elements to enhance nursing service efficiency. The results indicate that precision care nurses, under the interdisciplinary collaboration model, can integrate more resources to provide comprehensive precision care ranging from prevention and intervention to recovery, thereby improving patient satisfaction and reducing medical costs. Additionally, this model will also promote further development and optimization of the nursing profession, providing patients with comprehensive, full-process, and precise care.
